The Well-being Star has been designed for people living with a long term health condition, to support and measure their progress in living as well as they can. It can work as a stand-alone tool, or as part of Personal Health Plan materials. The Well-being Star is designed to either be self-completed by a patient, or ideally completed by a patient and health professional together.

The Well-being Star was developed by Triangle in collaboration with North East Essex PCT and with funding from the Department of Health. A 2nd Edition was published in July 2018, which included updated guidance for workers, specific guidance for service users being supported to complete the Star themselves, and clearer and more consistent language throughout.

The Well-Being Star supports and measures progress in eight outcome areas that together enable people to live as well as they can with a long term health condition. These are:

  1. Lifestyle
  2. Looking after yourself
  3. Managing symptoms
  4. Work, volunteering and other activities
  5. Money
  6. Where you live
  7. Family and friends
  8. Feeling positive

The Well-being Star scales are based on a five-point Journey of Change:

  1. Not thinking about it
  2. Finding out
  3. Making changes
  4. Getting there
  5. As good as it can be

Full resources are available for organisations with a Star Licence and training for all workers and managers using the Star.
